Players are required to arrive ten (10) minutes prior to the sessions (unless notified otherwise) to give sufficient time for preparation, this is to ensure that each session is fully utilised. Proform Basketball Pte. Ltd. (interchangeably known as the “Company”) has the right to video and/or photograph the training sessions and/or games for monitoring and assessing the player’s improvements and/or for promotional use in relation to the Company and its services. All rights including but not limiting to photographs, video footages and statistics are solely owned by the Company. As practices, games, and camps are scheduled, any missed sessions cannot be rescheduled and/or refunded. Please do notify the coach if you are to miss any games or practices. If any sessions are cancelled/postponed by the coach (or government directive), the session(s) will be rescheduled and made up at a later date. Please note that there will be no refund, but any cancelled sessions will be made up. The coach has the right to permanently remove any player from the training session, game, or any other event if the player’s behaviour is unacceptable. For the avoidance of doubt, there will be no refund if so. It is understood that the coach and/or Company are not medical doctors (or medical institution) and will rely on the player’s representations and disclosures regarding his/her health and physical condition. This consent form covers all present and future events that will be held by Proform Basketball Pte. Ltd. I believe that my child is in good health and is physically able to participate in the basketball program. By signing (ticking) below, I acknowledge and agree that my child has no limiting health conditions that would preclude participation in the basketball program, and will immediately inform the coach if any health condition arises during their participation in the training program. I agree to waive any rights, claims for damages, injuries, disability, death, or loss that may occur as a result of my child’s participation. This document releases and discharges all employees, volunteers, and/or Proform Basketball Pte. Ltd. itself from any claims or actions arising. Register
